How to get pregnant with PCOS
Women with PCOS can become pregnant naturally or with medical assistance. Up to 70% of women with PCOS are diagnosed with infertility.1,2 However at least 1 in 3 women with PCOS are fertile.3
For this reason, doctors usually recommend women try to conceive naturally for 6+ months if their cycles are sometimes regular or there is room for improvement (lifestyle modification).
However, if the couple are still unsuccessful after 6 to 12 months, the doctor will recommend medically assisted reproduction.
Natural Conception with PCOS
Many women with PCOS are able to get pregnant naturally however this does take longer than normal fertile couples.
Regular menstrual cycles ensures good ovulation, sperm friendly cervical mucus and a receptive endometrium.
Even though women with irregular cycles can still get pregnant, especially if they’re tracking ovulation, this is not usually recommended on its own.
Instead, experts recommend women trying to get pregnant naturally with PCOS consider:
- Exercise
- Diet
- Losing weight
- Minimize stress
- Eliminate endocrine disrupting chemicals
- Fertility awareness-based methods (FABM)
- Supplements
Of course this is still only possible with good sperm quality. Research suggests men need to have average sperm motility or TPMSC (total progressive motile sperm count) given women with PCOS have suboptimal cervical mucus.4,5
After successful fertilization, the embryo is then ready for implantation. However this can also be difficult for some women with PCOS due to endometrial dysfunction.6 These women fail to get pregnant naturally after 6 to 12 months of consistent ovulation or experience early miscarriages. In such cases, medical assistance is required.
Exercise
Exercise is a first-line treatment option for women with PCOS.7 Exercise (via irisin) decreases oxidative stress and inflammation, which in turn improves ovary function, hormone levels and insulin resistance.8,9,10
Exercise is reportedly more effective than diet at improving insulin levels.

However not all exercise is the same. In a large comparison study, high-intensity interval training (HIIT) significantly improved sex hormone-binding globulin (SHBG), free androgen index (FAI), insulin sensitivity and menstrual cycles more than medium-intensity interval training (MICT) among overweight women with PCOS.11 Patten et al. reported that HIIT participants were 7.8 times more likely to report improvements in their menstrual cycle.10 However, only low volume HIIT appears to improve fertility.12
